Failure Modes and Effects Analysis (FMEA)

Failure Modes and Effects Analysis (FMEA) is a procedure for analysis of potential failure modes within a system for classification by severity or determination of the effect of failures on the system.

The DP FMEA philosophy is to gain a clear understanding of the safest mode of operation for the vessel. Main objectives of the DP FMEA are to

  • Understand the worst case failure
  • Understand the vessel’s station keeping capabilities following the worst case failure
  • Identify significant single point failures ( if any) of the DP system
  • Identify the configuration for the safest mode of operation
  • Validation of technical suitability
  • Ensure contingency plans or operational interventions are in place

It is a general requirement of many national and international standards that FMEA is used in evaluating product integrity. FMEA is also one of the most widely used methods in system reliability. FMEA are normally carried out in a top down method and can identify failure modes in a system. Once this is completed, a FMEA can be used¬¬¬ to improve overall reliability and avoid failure and determine how these critical failures can be minimized or eliminated.  

By establishing the various failures, these scenarios can be tested out in proving trials to confirm the results. Recommended actions or compensating provisions are then made to reduce the likelihood of the occurrence and mitigate the risk.

  1. Aids in the initial design & helps determine changes to design specification
  2. Develops a list of potential failure modes ranked according to their effect on the customer. Establishes a priority system for design         improvement
  3. Helps determine if the level of redundancy is sufficient

  5. Assess the effects of the failure and resultant effect to the end user

The purpose of the DP Annual trial is to improve safety and performance by reducing risk. The DP Annual Trial ensures that the DP system is maintained in good working condition by systematic testing of all important systems and components of a DP system. Focus is placed on verifying the conditions from the failure modes and effects analysis (FMEA) that are difficult or impossible to verify by any other method. Periodic and managed testing will ensure that within a five year period, all FMEA test programmes/procedures will be completed.
